As you know, many of these health plans offer a variety of product lines. We may not participate in some of these (e.g. CDPHP Child Health Plus). Moreover, there are many smaller plans that we do accept that are not listed above. If in doubt, please touch base with us.
We also accept indemnity, select, and point-of-service plans.
Office Visit Fees
$75 for 10 min appointment (earaches, sore throats, rashes...)
$150 for 20 min visit (check up on healthy child, headache, fatigue, and other complex concerns)
Payment is expected at the time of service [and a discount will be provided - contact us for current rate].
These fees do not include office labs (rapid strep, urine analysis), office tests (vision, hearing screens), vaccines, or billing fees. We accept Visa, Mastercard, Discover credit cards, check, and cash payment along with HSA and FSA cards.
We recognize that increasing numbers of families either have no health insurance, or are moving toward high-deductible health plans (HDHPs). Note that effective January 1, 2011, a $50 deposit will be required at the time of visit for patients being seen with HDHPs.

As a convenience for our patients, you may now care for an unpaid balance with our practice using PayPal. Follow this simple steps:

- Phone consultations with the MD or PNP during or after business hours
- Email consultation with the MD or PNP during or after business hours
- House calls (including travel time)
- Travel research; researching specific concerns; letters / phone calls to consultants
- Managed care negotiations (phone calls to medical director)
We currently bill the above uncovered services at $300 per hour for MD, $200 per hour for PNP. We also offer the following menu of uncovered services:
- Ear Piercing (by PNP) -- $75
- Application of LMX before injections and vaccines -- $10-15
- Medication (other than controlled substances) refill w/ brief phone consult -- $25-50
- Referral to specialist without an appointment w/ brief phone consult -- $25-50
- Forms – school, camp, daycare, disability, special services -- $10-25
- Prior authorizations for imaging and non-formulary drugs without appointment -- $25-50
As a convenience for our patients, you may now care for an unpaid balance with our practice using PayPal. Follow this simple steps:
- Verify your balance, either on the last statement or by calling our office.
- Hit "Pay Now" below, you will be taken to the PayPal website.
- Enter balance amount in the field "item price." Then hit update. Then log in to your PayPal account.
